I, [Magick Name, Motto or your Full Legal Name], do hereby swear the following Oath, in the Presence of the Order of Angels OPHANIM, the Anchangel RATZIEL and the Lord of the Universe, who art known in CHOKHMAH by the name YAH: to Prosecute the Great Work of a MAGUS, of Hermetic Order of the Round Table: which is, at this level of spiritual development, to make my every Act an expression of my Magick Formula.
